Disability Claim Information 

 

There is a $20 fee for EACH initial disability claim (State of CA, private insurance companies [i.e., Aflac, Unum, Colonial Life, Standard, Aetna, etc.]). Leave of Absence forms cost $10 each. This includes all patients, spouses, or care providers for pregnancy leave or GYN surgeries. Claims for GYN surgeries will be submitted within 5 days after your procedure is performed since correct diagnosis and procedure codes are required. There is a $10 fee for each disability extension completed. Prepayment is required before completion. We now take phone payments (Visa/M/C) for this service.

 

All disability claims, leave of absence forms & extensions are filed within five businesses days of receipt in office.
